What We Do

Village Family Health Team (VFHT) is a team of health care professionals providing a full range of primary health care services to the residents of Liberty Village, West Queen West, King St. West, Parkdale and Dundas St. West. The Village Family Health Team is funded by the Ministry of Health and Long term Care.

Helping You Stay Healthy

A Family Health Team is a new way of providing primary health care in Ontario. It is a team approach that brings together family physicians with other health care providers in order to coordinate the best possible care for patients. Everyone on the team works together to meet your needs. Our focus will be on keeping you and your family healthy, not just treating you when you are sick. You and your family will benefit from a comprehensive approach to health care.

When you enroll as a patient with our Family Health Team you are taking a step to ensure that you always have convenient access to regular check-ups and ongoing healthcare from a family physician and/or team of other healthcare providers.

Your primary care team at Village FHT consists of:

• Nurse Practitioner
• Registered Nurses
• Social Worker
