Village Family Health Team's (FHT) doors are open! We are now operating out of our
newly constructed space, located at 171 East Liberty St, in Liberty Village (south
west Toronto)!

Village FHT provides a full range of primary health care services through an
interprofessional team including: Doctors, a Nurse Practitioner, Registered Nurses,
a Social Worker, a Clinical Pharmacist and a Registered Dietitian.

All of our physicians are currently accepting new patients. If you need a family
