Every month our physicians receive a report from the Ministry of Health that identifies every visit patients make to another primary care provider such as a walk in clinic or emergency department for a non- emergency issue. When patients use outside services, their own doctor is penalized financially. Please contact us via WellX, telephone or the website if you need to see your doctor. We are here to help you stay healthy!

Village Family Health Team's (FHT) is Located at
Unit 102
171 East Liberty St, in Liberty Village (south
west Toronto)! We are located on the south side of East Liberty Street between Origin restaurant and The Dollar Store
Village FHT provides a full range of primary health care services through an
interprofessional team including: Doctors, a Nurse Practitioner, Registered Nurses,
a Social Worker, a Clinical Pharmacist and a Registered Dietitian.
Some of our physicians are currently accepting new patients. If you need a family
