Advanced Access
Effective May 13 2013, Drs. Don Smith, Charles King and Cristina de Lasa will be moving to Advanced Access for patients appointments. This means patients will be able to see their doctor the day they call or the next clinic day. Please read more about Advanced Access on our website.

Village Family Health Team's (FHT) is Located at

Unit 102
171 East Liberty St, in Liberty Village (south
west Toronto)! We are located on the south side of East Liberty Street between Origin restaurant and The Dollar Store

Village FHT provides a full range of primary health care services through an
interprofessional team including: Doctors, a Nurse Practitioner, Registered Nurses,
a Social Worker, a Clinical Pharmacist and a Registered Dietitian.

Some of our physicians are currently accepting new patients. If you need a family
