At Village Family Health Team, our mission is to provide comprehensive, compassionate, and accessible healthcare to our community. We believe in a holistic approach to health, where the physical, mental, and social well-being of our patients are prioritized. Our dedicated team of interprofessional healthcare providers works collaboratively to deliver personalized care that meets the unique needs of each individual and family we serve. We operate under a family health team model, this means that as a patient you will be rostered to a primary family doctor or nurse practitioner but you will also have access/and see other members of our healthcare team including nurses, nurse practitioners, social workers, pharmacist, dieticians, chiropodist, and other physicians.
